Form. Call us at 425-562-8840 ext. 229 and we'll send
you one. Or get it online by right-clicking on this
link and selecting "Save Target As..."
(Internet Explorer) or "Save Link As..." (Netscape).
Save the Word document onto your computer and fill it
out. Also save and fill out sections C and D of the
State Request For History
form. Print out and sign both forms, then mail them
to:

Bellevue, WA 98008-0398
The Volunteer Coordinator will contact you as soon
as we receive your application. All applications are
kept on file for the next Volunteer Training session.
You will be notified as soon as the date for the next
training is set.
